What is complex care?
Complex care is a type of care service which supports a person who has substantial and ongoing health needs, usually with a combination of live-in and visiting care services. Complex care is also known as nurse-led care or continuing care.
Complex care is a holistic care service, providing support in all aspects of the physical, clinical, cognitive and social needs of our clients. My Care World lead nurses offer support with complex medical needs, and our professional care assistants provide a one-to-one service enabling complex care clients to remain living in their own homes with dignity and independence. From heartfelt companionship and support with light domestic tasks, to specialist care and bespoke support for medical conditions, complex care is a holistic and beneficial service for our clients with ongoing needs.

We at My Care World, we’re passionate about ensuring that our clients are able to continue living in their own homes with dignity and independence. Our nurse-led home-based healthcare services are overseen by lead nurses and supported by a team of specially trained and highly skilled MCW Carers.
We work in partnership with our clients’ clinicians and other advisers (such as clinical commissioning groups, local authorities and the wider NHS as well as with private individuals and independent case managers) to ensure that our care is entirely tailored to the medical needs and personal wishes of the individual.

MCW Value for your money
Our services are competitively priced and represent a substantial saving for commissioners in comparison to caring for individuals in residential environments. We take into account the complexity of the individual care package and the specialist requirements from our team. Home-based care provision is both more cost-effective and a strong preference for clients and their families.
Enquiry and assessment
When we receive a referral or an enquiry, we will make an appointment to meet the client and their family at the earliest opportunity. During this appointment, our lead nurse will carry out a detailed risk and needs assessment. We use this assessment as a basis for estimating the cost of providing care but, most importantly, as the initial basis for the client’s individual care plan.
At the earliest possible stage, MCW will introduce our dedicated lead nurse who will lead and oversee the client’s care and be the primary point of contact for all aspects of our services.
Once we have agreed requirements with the client, the client’s family and the commissioner, we then begin organising and establishing the provision of care, including the following:
- Training of a team leader who acts as a key worker within the direct care team;
- recruitment of care staff that are matched to the specific needs of the client and trained accordingly under the supervision of our clinical specialists;
- Introductions of care staff to the client and family members as appropriate;
- Identification of specialist care needs and matching with the appropriate healthcare professional;
- Establishment of an efficient and consistent rota; and
- Access to 24-hour on-call support services
Individual care plan
An individual care plan is the centrepiece of the complex care service. We take the time and the necessary diligence to ensure that the clinical and the personal aspects of our service are documented and agreed in conjunction with the needs of the client ( as well as the interests of family members, existing carers, commissioners and other healthcare professionals that are involved). Preparation of the care plan starts with spending time in the home or, where applicable,
Once the care plan and funding has been agreed, we will prepare a client contract to outline roles and responsibilities.

There are various documents and papers that will be prepared for each client and provided to the client and/or family members upon commencement of services, as follows:

Regular review of MCW services
One of our dedicated managers will visit the client regularly and contact them every month to make sure they’re receiving the best quality and level of care.
Which conditions does nurse-led care usually support?
Any complex care client has a unique set of requirements and needs. At My Care World, we see everyone as an individual with their own needs and wishes, and our care is designed to be entirely bespoke. We ensure that our services are clearly outlined in an individual care plan, which is regularly reviewed and updated. Visiting care, live-in care, and a combination of the two, are all entirely flexible services to accommodate for changing needs and wishes over time.
